Forms Packet

Special Instructions: Black Hawks Volleyball has a group of 4 required documents for parents to complete prior to participating in our programs. They are listed below. We are working on a new Club Administration program that will gather this information electronically during your initial registration. It was anticipated to be ready by October. Because of delays in delivering this feature we will need each player to attend clinics and tryouts with each form in our “Forms Packet” printed, each line filled in, and signed by both the Parent and Player in required fields. Please hand deliver these forms to our Club Director.

After our new system goes online we will have everyone go back into their profile and update this information electronically and then print the forms that will be automatically populated. Once you have completed this information you should never have to complete these forms with us again. At any time after the initial information is entered you can go in to your profile to update information as needed, such as change of address, or change of contact information.
